I downloaded this file from somewhere in June 2010 and although the data may not be all that uptodate currently, I am placing it here for what its worth. It has also been included in one of the discussions on RHD

I did a search and found the latest revised (February 2012) of the guidelines at

I am replacing the previous file with this latest more extensive file
